Why patients do not turn up, and what a reminder text should say

A no-show is usually not a patient who does not care. It is a patient nobody reminded, and a patient who had no easy way to tell you she could not make it.

Why she does not turn up

Ask any receptionist and the reasons are always the same handful. She forgot. She was not sure whether it was Tuesday or Wednesday. Something happened at work, or with a child, or with the fare that week. Or the tooth stopped hurting and the reason to come felt like it went away with the pain.

Almost none of those are a patient who does not respect the clinic. They are the ordinary friction of a busy life, and every one of them can be caught by a message the day before.

The worst part is the silence

The empty chair is the small loss. The real loss is that she never told you, so the hour could not go to anybody else and nobody knew to call the person who wanted an earlier slot.

She does not cancel because cancelling feels like a confession. She would have to say she cannot make it, possibly to someone who sounds disappointed, and it is easier to say nothing and feel guilty quietly. A lot of patients who no-show never come back at all, because now there is an awkward thing between her and the clinic.

Make it easy for her to say no, and most of the time she will say when instead.

When to send it

Two messages do almost all of the work. One when the appointment is made, so she has the date in writing and can look it up. One the day before, in the early evening, when she is on her phone and can still move things around at home or at work.

A same day reminder a few hours before helps for morning appointments, and it is the last chance to hear a no while the slot can still be used. More than that starts to feel like nagging, and a patient who feels nagged stops reading.

What the text should say

Short, specific, and with an easy way out. It needs the clinic name, the day and time, and one tap that lets her change it without a conversation.

Hi Maria, this is Santos Dental Clinic. We have you tomorrow, Tuesday the 12th, at 10am. If you need a different day just tap here and pick one. Salamat po.

The link matters more than the wording. If her only option is to call the clinic and explain herself, she will not, and you will find out at ten past ten.

The tone that works

No guilt. No reminding her that the slot was reserved for her. No warning about a fee. Every one of those makes the silent no more likely, not less, because they make getting in touch feel expensive.

Use her name and the clinic's name. A message that just says you have an appointment tomorrow at 10 reads like something automated and gets swiped away with the promos.

Should you charge a no-show fee

For most small clinics here, no. It reads as a threat to a patient who is already nervous about cost, it is hard to collect, and it costs you the relationship with someone who might have come back for years.

The exception is the long appointment that blocks the chair for hours and turns away other patients. If you take a deposit for those, say so kindly and say it when the booking is made, never in the reminder the night before.

What to do after a no-show

Send one message. Not three, and not one that asks why. Say you missed her, say you hope everything is alright, and offer to find her another time.

Hi Maria, we missed you this morning at Santos Dental Clinic. No problem at all. If you would like another day, tap here and pick one that suits you.

That message quietly removes the awkwardness she was expecting, and a good share of no-shows book again from it. Left alone, most of them never come back and nobody at the clinic ever knows why.

Where to start this week

Take tomorrow's list and text every name on it tonight, by hand, from the clinic phone. Then do it again the next night. Within a week you will see the difference in the morning list and you will not need convincing after that.

Doing it every single night, for every patient, without anybody having to remember, is the part that stops happening in a busy clinic. That is the part we take over.
